To make your first visit as smooth as possible, please review the information below.
Your First Visit + Consultation
Your initial appointment is a focused consultation where we review your diagnosis and discuss your customized treatment options.
Consultation Timing: We can occasionally perform surgery on the same day as your consultation. However, if your case involves a complex medical history, an extensive treatment plan, or the patient wishes for sedation, we will require a separate appointment for surgery to ensure the best possible preparation and outcome.
Patients Under 18: All patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation visit.
What to Bring to Your Consultation
Please help us prepare for your visit by bringing the following documentation:
Your surgical referral slip and any applicable X-rays.
A complete list of all medications you are currently taking.
Your medical and dental insurance information and any necessary completed forms. (Providing these ahead of time allows us to expedite your claims processing.)
Important Medical Information
X-Rays
If your general dentist has taken recent X-rays, please arrange to have them forwarded to our office before your consultation. If there is not enough time, please bring them with you. Should additional films be necessary for your treatment plan, we can take them at our office.
Using Anticoagulants (Blood-Thinners)
For patients taking anticoagulants (e.g., Warfarin/Coumadin, Plavix, Aspirin, Eliquis, and Xarelto) to prevent clotting, these medications can increase the risk of bleeding during surgery. If you take Coumadin (Warfarin), you must have an INR blood test a day before your procedure so we can safely manage your care and allow you to continue your essential medication. Please discuss your specific medical circumstances with your surgeon.
Pre- and Post-Surgical Instructions
We provide general surgical instructions on our website for the convenience of our patients. As always, your surgeon will provide instructions for your specific procedure and individual circumstances at your consultation.

Before your first visit, please complete our new patient registration forms. When filling out these forms, you will need the following information on hand:
Insurance cards (dental and medical)
Medication list
Referring dentist(s) details
Primary care provider name
