Payment Policies and Options
Payment is expected at the time services are rendered. We accept the following forms of payment:
Cash and Money Orders
Visa, Mastercard, and Discover Credit Cards
Third-Party Financing: We proudly partner with CareCredit and LendingClub to offer special financing options that can make your procedure more affordable.
Insurance Information
We work closely with you to ensure you receive the maximum reimbursement for your covered procedures. Please bring your insurance information to your first office visit to expedite reimbursement.
Participating (In-Network) Plans
Out-of-Network and Other Policies
Out-of-network
For all other insurance plans, we require payment in full at the time of service. As a courtesy, we will submit the claims to your insurance company, and your reimbursement will be sent directly to you.
Medicare Update
Please note that as of October 1, 2024, our providers have opted out of Medicare.
Important Responsibility Note
Please remember that you are fully responsible for all fees charged by this office, regardless of your insurance coverage. Insurance policies vary greatly, and your policy's fixed allowances may not always align with our usual and customary fees.
Cancellation and No-Show Policy
We appreciate your understanding that appointment times are limited and value your cooperation in helping us serve all our patients efficiently.
If you must cancel your appointment, we respectfully request a 24-hour notice.
Appointments missed or canceled without 24 hours' notice will incur a $50 fee.
